Transcript and Credit Information
On November 20, 2023, you received a reminder email to complete your Conference evaluation. This evaluation is required in order obtain your Continuing Education certificate.
Click this link and follow the instructions to complete the Cloud CME evaluation and obtain your certificate: https://hopkinscme.cloud-cme.com
To receive credits, all attendees must attest to the time spent in the activity.
On-Demand Course Completion: Not all course components may show as “Complete” in the online Conference platform. This issue does not impact your eligibility for CE/CME. You must self-attest your CE/CME hours at the end of the event.
The deadline to receive ASHA credit is November 25 (you must complete the Evaluation by November 25, 2023.) No exceptions can be granted.
For all others are seeking credit other than ASHA, the evaluation and attestation will expire on December 17, 2023 after which we are unable to process your certificate.
Please be sure to access prior to expiration as per below:
- Click Sign-In
- Sign-In using your email address and password (or JHED ID and password if you are Johns Hopkins staff).
- Click on My CME
- Click on Evaluations and Certificates
- Click “Complete Evaluation” in the row for Twelfth Annual Johns Hopkins Critical Care Rehab Conference.
For Physical Therapists only: On the evaluation and credits screen, when selecting a certificate for download, please select any PT state board that is listed. All of the PT approval statements are listed on each certificate.
Once submitted, you will be directed to a brief survey to offer your Conference feedback.
After the above, your certificate will be immediately available for print or download.
